Opt-in or Opt-out of Identity Protection Services
Because the offer may result in taxable income to your employees and create tax reporting obligations, you have the option to decline these new identity protection services for your employees. We are asking that you specifically opt-in or opt-out of this service. Before making a decision, we strongly encourage you to consult with your tax advisors. If you decide to opt-in, your employees will receive a letter by January 1, 2016 describing the service with instructions on how to enroll. If you decide to opt-out, we will not extend this offer to your employees.
When you are ready to either opt-in or opt-out of this service, please inform us by filling out a brief survey. You will need your account number to fill out the survey.
Please access the survey here: http://www.bluecrossma.com/id-protection.
Note–Please either opt-in or opt-out no later than November 6, 2015. Accounts that do not respond will be automatically opted-out of the service.
