Healthy Actions Program
Rewards for Employees, Rewards for You, Too.
Wellness programs have become a smart way for employers to encourage healthy habits in their employees. And, as we all know, healthier employees are happier and more productive. That's why we're introducing Healthy Actions—your solution to a healthier, happier workforce.
Healthy Actions is a unique wellness program available to small group employers with 10-50 employees enrolled in a qualifying plan. This program rewards employees for taking an active role in their health. And based on employee success and participation rates, employers get rewarded with up to 5 percent of their premium.
Employee Rewards
This program is highly attractive to employees, because it rewards them up to $300 for taking an active role in improving their health. Here's how it works:

| Is healthy | Receives a $300 Visa debit card |
| Needs to improve their health (for example, lose weight, take medication to lower cholesterol, etc) | Receives a $100 Visa debit card Then the employee needs to work on the health goal set by his or her doctor within a set timeframe And, if the employee meets the goal and submits a second form before plan renewal, he or she will receive an additional $200 Visa debit card |
Rewards for You, Too
Employees aren't the only ones benefiting from the Healthy Actions program. Since you're making an investment in your employees' health, if they succeed, you also receive a financial reward, based on employee participation and success. And the best news: the more your employees participate and become healthier, the bigger your reward.
Employer Rewards
The financial reward you receive depends on the percent of employees who complete the program by earning the full $300 reward.

| 50-79% | 2.50% of current year's premium |
| 80-100% | 5.00% of current year's premium |
Healthy Actions is available to qualifying new accounts and existing accounts on plan renewals beginning on August 1, 2012.
