Introducing a New Plan for Large Groups (51+)

Beginning January 1, 2013, we will be offering a new affordable option: HMO Blue New England $3,000 Deductible^SM

Introducing HMO Blue New England $3,000 Deductible
We are pleased to announce a new affordable HMO Blue New England plan that combines New England-wide coverage with the savings of our HMO Blue New England regional managed care plans. This new offering includes:

A $3,000 individual and $6,000 family deductible
$25 primary care provider and $40 specialist copayments
Value-Based Benefits enhancement as a core benefit feature for large group accounts with 99 or fewer enrolled
The option to include Hospital Choice Cost Sharing via rider for additional savings

Must Be Paired with an HRA
In order to offer this plan design, the account must attest that the plan has been paired with a Health Reimbursement Account (HRA) that is funded to at least $1,000 individual/$2,000 family but not to more than 50% of the deductible amount. All members under the plan must be covered under the HRA. The HRA can be funded through one of our preferred vendors or through the vendor of the account's choice. For more information about pairing this plan with an HRA, please contact your account executive.

NOTE: The HMO Blue New England $3,000 Deductible plan design, on its own, does not meet Massachusetts Minimum Creditable Coverage (MCC) but does meet MCC with the HRA account.

Available to large group (51+) insured and self-insured accounts effective January 1, 2013.
