Upper GI Prior Authorization Delay
In the 
June issue of Important Administrative Information (IAI), we notified you that effective October 1, 2009, upper GI endoscopies and shoulder arthroscopies performed in an outpatient setting will require prior authorization for commercial HMO/POS products. Please note that the prior authorization requirement for upper GI endoscopies will not be effective until November 1, 2009. Prior authorization for shoulder arthroscopies will still be required beginning October 1, 2009, as previously communicated.
