• Platinum Plan

  This plan has the highest monthly premiums but the lowest out-of-pocket costs for medical expenses.

  HMO Blue Premium SBC
  

  Coverage Policy Document

• Gold Plan

  This plan generally has a higher monthly premium than the Silver plan and offers a good balance between lower out-of-pocket costs and a higher monthly premium.

  HMO Blue $1,000 Deductible SBC

  This plan generally has a higher monthly premium than the Bronze plan. You' wi'll have moderate out-of-pocket costs when you receive
  medical care.

  Access Blue Basic SBC

• Bronze Plan

  This plan offers the lowest monthly premium. However, you pay higher out-of-pocket costs when you receive medical care.

  HMO Blue Basic Deductible SBC

• Catastrophic Plan

  This plan is for individuals under 30. You're covered for medical care at a low monthly premium.

Prescription Coverage

Prescription drugs are grouped into tiers based on type and cost. Your out-of-pocket cost under each plan's prescription drug coverage can differ depending on which tier a medication falls under. Use our Medication Look-Up tool to find out how our plans will cover your prescriptions.
