Understand Your Claim Summary

As a member, you may have received a statement in the mail called a Subscriber Claim Summary or Explanation of Benefits. This statement explains how your benefits are handled.

The Subscriber Claim Summary includes:

• The name of the health care provider who sent us the claim
• The date of service
• The type of service provided
• The amount the provider charged for the service
• How much you share in the cost of your health care

It's important to note that this statement is not a bill, and you should not send any payment to us.
