Things You Should Know About Medicare HMO BlueSM ValueRx

  • The Medicare HMO Blue ValueRx plan provides coverage for your Original Medicare Glossary benefits plus additional benefits that Medicare doesn't generally cover including Part D prescription drug coverage. You only need to show your Medicare HMO Blue ValueRx card when you get care.

  • You get routine care from a network primary care provider Glossary.

  • Your primary care provider coordinates your care with specialists, hospitals, and other network providers.

  • This plan includes routine preventive dental, vision, and hearing benefits.

  • Our Medicare Advantage Plans earned 4.5 Stars out of 5 Stars for Overall Plan Rating and 5 Stars for Customer Service according to the Centers for Medicare and Medicaid Services' (CMS) ratings for 2014.1

You are eligible to enroll if...

  • You are entitled to Medicare Part A and enrolled in Part B.

  • You permanently live in Barnstable, Bristol, Essex, Franklin, Hampden, Hampshire, Middlesex, Norfolk, Plymouth, Suffolk, or Worcester counties in Massachusetts.

  • You do not currently have end-stage renal disease. If you initiated dialysis treatments for end-stage renal disease but have recovered your normal kidney function and no longer require a regular course of dialysis to maintain life, or have had a successful kidney transplant, or are currently a member of Blue Cross Blue Shield of Massachusetts, you may still join the plan. In addition, if you were a member of a Medicare Advantage plan that terminated its services after December 31, 1998, and you currently have end-stage renal disease, you may still join the plan. There may be additional requirements. Please contact the plan for details.
