You can obtain a copy of your Summary of Benefits or Evidence of Coverage (EOC) by choosing one of the links below. Just select your plan or the one you're interested in to download and review a copy. If you aren't sure which plan you're enrolled in or would like a copy of your EOC mailed to you, please call Member Service at 1-800-200-4255 (TTY: 1-800-522-1254), Monday through Friday, 8:00 a.m. to 8:00 p.m. ET. For Medex plans please call 1-800-258-2226 (TTY: 1-800-522-1254).
