Weight Loss Benefit

Managing your weight is an essential part of staying healthy. That's why Blue Cross Blue Shield of Massachusetts provides up to $150 toward fees paid for a qualified Weight Watchers®'' (Weight Watchers Traditional or Weight Watchers at Work programs only) or hospital-based weight loss program.

How Do You Know If You're Eligible?
Most members in HMO Blue®, Blue Choice®, Blue Care® Elect, Managed Blue for Seniors, Medex®' Core Plus, Medicare HMO Blue, Medicare PPO Blue or Blue Medicare PFFS plans will qualify, upon renewal in 2004, for the Weight Loss Benefit. Please check with your benefits administrator (or call the customer service number on your ID card) to ensure that you are eligible.

Exactly What Kind of Weight Loss Programs Qualify?
In addition to Weight Watchers Traditional or Weight Watchers at Work programs, the Weight Loss Benefit provides up to $150 per membership (i.e., family or individual) per calendar year for fees paid for hospital-based weight loss programs offered by and held at licensed hospitals. These do not include programs offered at specialty clinics, health centers, or health spas.

The Weight Loss Benefit does not cover you for individual nutrition counseling sessions, food, books, videos, scales, or other items not included as part of a qualified program's course fees.

BE SURE TO CHECK WITH YOUR PHYSICIAN BEFORE GETTING STARTED IN A WEIGHT LOSS PROGRAM.

How to Apply For Your Weight Loss Benefit:
After you confirm that you are eligible for the Weight Loss Benefit and you have enrolled in, and paid dues for, a qualifying program, simply

send:

  1. A completed: Weight Loss Benefit Form | Weight Loss Benefit Form (Spanish)

  2. 8.5" x 11" photocopies of paid receipts from the qualified program in which you enroll.

  3. Receipts must show BCBSMA member's name, name/logo of program, amount paid for session(s), and date(s) paid. For qualified Weight Watchers® programs, a photocopy of your program "Membership Book" showing this information is required.
