Clinicians

As a clinician, you play a very important role in the lives of the youth you serve-especially when it comes to promoting behaviors that support good health. The office visit gives you an excellent opportunity to screen and classify BMI, counsel families on the importance of healthy eating and active living, and when needed, coordinate participation in school and community based nutrition and physical activity programs. Your message can be as simple as "5-2-1."

  • 5 servings of fruits and vegetables per day
  • no more than 2 hours of television, video games, or computer time per day
  • at least 1 hour of physical activity per day

The Clinician's Toolkit

The Jump Up & Go! Toolkit provides materials to help you promote good nutrition and physical activity. For example, you can:

  • Use the Physical Activity and Nutrition Survey to explore the current lifestyle behaviors of your patients and their families.


  • Ask your patients to complete the Jump Up & Go! Weekly Log, which can help families track their efforts and progress as they seek to increase their consumption of fruits and vegetables, decrease screen time, and increase physical activity.


  • Launch a Jump Up & Go! challenge among your patients to encourage them to complete and return their Weekly Log.
