We offer specialized support services for certain conditions through our comprehensive group of Blue Care Connection® Case Management programs.
Our programs are designed to support individuals with more complex care needs associated with severe illness, injury, or other conditions. Participants are identified for the case management program through a combination of claims data, physician referral, and self-referral. A specially trained registered nurse offers specialized support for each Blue Care Connection® program.
Examples of case needs that may be appropriate for case management support include:
- Active cancer treatment or advanced colon, lung, or prostate cancer
- Acute neurological disorder
- Congenital anomalies
- Gastrointestinal disorders
- High-risk pregnancy
- Infectious disorders
- Multiple trauma
- Pediatric disorders
- Severe endocrine disorders
- Spinal cord injury
- Traumatic brain injury
We offer targeted case management for the following conditions:
