Cancer can affect anyone at any age and comes in all forms and levels of severity. People with cancer can often feel overwhelmed with the diagnosis and feel lost in a health care system that offers conflicting answers to their multiple questions. It is hard to know how to sort through all the information, recommendations, and proposed treatments. Our Oncology Care Case Management team can help guide you whether you have a new diagnosis of cancer or an oncology diagnosis with metastasis disease.

Some of the types of cancer we can help with are: leukemia, lymphoma, and cancers of the lung, colon, prostate; ear, nose and throat; endocrine glands; bone and connective tissue, head and neck; breast; female genital tract; genitourinary system, hepatobiliary system; central nervous system; stomach and esophagus.

The program can help members:

  • Understand their overall plan of care
  • Coordinate care among the members of their treatment team
  • Identify health goals specific to treatment
  • Identify and use resources, including transportation, financial support, and community services
