Your search will provide you with a description of your medication, tier, limits/restrictions, and additional information regarding the drug.

The amount you pay—your copayment level—is determined by the tier structure indicated on your ID card.

To determine if your plan design includes BlueValue Rx, please refer to your identification card and look for the BlueValue Rx logo. If your plan design includes the Standard Pharmacy Program Formulary, please select the Standard Pharmacy Program Formulary tab below.

New Member ID Cards

We have recently updated our member ID cards. When you recieve your new card some time this year, remember to provide the card to your physician(s) and pharmacies and ask that they update their records.

The new ID cards have additional space on the black magnetic strip on the back for storage of additional information for your health care providers. Please refer to your benefit literature for information about your pharmacy copayments or call the Member Service number printed on the front of your ID card. You can also access your pharmacy copayment information by registering for or logging into Member Self Service.


Standard Formulary Medication Search Process

  1. Select Your Tier Structure. If you have 2 numbers after "RX", you have a 2-tier structure. If you have 3 numbers after "RX", you have a 3-tier structure. If you do not select a tier structure, your search will be defaulted to a 3-tier level.
  2. Enter Medication Name. Type in the first three letters of your prescription medication - exactly as it appears on your prescription container and click on "search".
