Once you've listed your providers, you should check your current providers online. You can search for them using the Find a Doctor and Find a Hospital/Facility links below. Record their names and tiers in the self-assessment form for future reference.
If you need help, please call the Member Service number on the front of your ID cardIdentification cards are provided to all participants for proper identification under their group health plan. ID card information helps providers verify patient eligibility for coverage.

Blue Options network recently improved their quality and cost scores. Beginning January 1, 2011, you'll be able to receive care at these hospitals for a lower cost because they belong to the new tiers listed below:
