About Hospital Choice Cost Sharing

If your plan includes the Hospital Choice Cost Sharing feature, you can control your share of medical costs for hospital care. That's because what you pay depends on the location you choose for your care.

How can I tell if my plan includes this feature?
There are several ways you can tell:

• Check your benefit materials. These would include an indication that the Hospital Choice Cost Sharing feature is included.
  
  
• Check your member ID card. When your plan renews or if you join after April 1, 2012, you will receive an ID card that includes an indication in the lower right portion of the card (see image at right). Keep in mind that your network or plan name listed on the card may be different than this example.
  
  
• Ask for help. Contact your plan sponsor (for most people, this is their employer), create an account or log into Member Central, or call the Member Service number on the front of your member ID card.
