1099-HC Forms
All 1099-HC forms to eligible subscribers will be post-marked by January 31, 2014. If you have not received a form by the first week of February 2014:

Members
If you have not received a form by the first week of February 2014, please:

  • Login to your Member Central account and click on "Contact Us." From there you can use our secure inquiry form to request a PDF of your MA 1099-HC form to be sent via secure email, or

  • Call Member Service at the number on the front of your medical ID card

FEP Members
If you have not received a form by the first week of February 2014, please:

  • Visit www.fepblue.org, click on MyBlue, log into eService, and click on Contact Us, or

  • Call the FEP member service telephone number on the back of your ID card

Member Service
Visit our FAQs section for answers to many common questions regarding MA 1099-HC tax forms, browser compatibility, changing your address, coverage information, flu facts, and forms. You can also access your account online, where you can:

  • Review your claims and benefits

  • Update your address

  • Change your primary care provider

  • Request a new ID card

  • Submit a question to Member Service using our secure Contact Us feature. Once logged in, follow the Contact Us link at the top right corner of the page. All questions will be responded to within one business day.

You can also call Member Service at the number on the front of your ID card, Monday through Friday, between 8:00 a.m. and 6:00 p.m. ET.

Order a new member ID card by calling this number Monday through Friday, between 8:00 a.m. and 6:00 p.m. ET. You can also replace your ID card online by visiting the My Account section.

Find a Doctor (outside Massachusetts)

Through the BlueCard® program, you can find a participating hospital or doctor outside of Massachusetts or the U.S. by calling this number 24 hours a day, 7 days a week. (Benefits are limited outside of the service area.)

Behavioral Health and Substance Abuse

Call the Behavioral Health department anytime, 24 hours a day, 7 days a week, to request covered services that require pre-approval.

Privacy
If you want to report a suspected privacy violation, please call 1-866-635-3114.

24-Hour Nurse Care Line / Blue Care Line

Mail Service Pharmacy

If you have prescription drug coverage, call this number anytime, 24 hours a day, 7 days a week, with questions about the Mail Service Pharmacy program.

Get a Cost Estimate
Find A Doctor will help you find high-quality, lower cost options for care in minutes. You can find doctors, dentists, hospitals, and other healthcare providers and get an estimated range of average cost for hundred of common medical services based on your health plan benefits. Start by logging in to Member Central. After logging in, click on Find a Doctor to search for healthcare providers or get cost estimates.

If you prefer a written estimate, log in to Member Central and click on the "My Account" tab. Next, click "Request a Written Estimate."
