The Point-of-Service (POS) plan combines the benefits of the HMO plan with the ability for members to self-refer for out-of-network coverage.
You also have the option to seek covered services from a covered provider who is not a network provider. (These health care providers are often called "non-network providers.") In this case, you usually receive the lowest benefit level under this health plan (your Self-Referred Benefits).
You must choose a primary care provider (PCP) for you and each member of your family.
This plan has a deductible of $250 per member and $750 per family.
Your in-network service area is New England (Massachusetts, Rhode Island, Vermont, Connecticut, New Hampshire, and Maine). If you travel outside this area, you are covered for an urgent or emergency care visit and one follow-up visit. To receive the highest level of benefits, any additional follow-up care must be arranged by your PCP. While out of the coverage area, you also have benefits for out-of-network services, including urgent care.
If while traveling you need routine care (checkups, vaccinations, etc.), you will be covered for out-of-network services.
When traveling internationally, you are covered through the BlueCard Worldwide network. This network lets you get doctor and hospital care from participating providers around the world.
