Standard and Basic Tiers: $250 for one member, $500 for two members, or $750 for a family

Your in-network out-of-pocket maximum: $2,500 for an individual or $5,000 per family (these amounts include your deductible)
You pay a copayment or co-insurance for certain services or prescription medications. By choosing Enhanced Benefits Tier providers each time you get hospital or PCP care, you can generally lower your out-of-pocket costs.
