Standard Option

The Standard Option provides comprehensive coverage combined with the flexibility to choose providers from inside or outside the preferred provider network. There are more than 350,000 preferred providers nationwide. Not only nationwide. You have a substantial preferred provider network right here in Massachusetts!

You never have to worry about obtaining a referral. Keep in mind, your out-of-pocket costs will be lower when you use preferred providers, but you do have the flexibility to use any covered provider.

Features of the plan include:

• Never, ever need referrals

• $20 per visit to primary doctor/$30 per visit to a specialist for medical evaluations

• No copayment for adult preventive care and well-child care to age 22

• Preventive (limited) fee schedule dental coverage for periodic exams, cleanings, fillings, X-rays, and extractions for covered adults and children

• Generic Incentive Program for Prescriptions - Preferred Retail and Mail Service - first 4 fills/refills are free when you switch from certain brand names to specific generics. Learn about our

• Prescription Drugs—Preferred Retail Pharmacies

  • 20% co-insurance for generics

  • 30% co-insurance for preferred and specialty medications

  • 45% coinsurance for non-preferred medications

  • $10 generics

  • $70 preferred brand

  • $95 non-preferred brand

   

• Specialty Pharmacy Program

  • $80 copayment specialty drugs (up to a 90-day supply)

   

• Special benefits include: 100% coverage for maternity care; 100% coverage for accidental injury treatment within 72 hours of accident

• Only certain services are subject to $350 person/$700 family calendar-year deductible*

• For members 18 years old and above, earn up to $50 on a MyBlue Wellness debit card by completing the Blue Health Assessment and three online coaching sessions (maximum of 2 cards per family)
